What each form is for

Agency Form Purpose
SSS R-1A Employment report: new employees and their SSS numbers
SSS R-3 Contribution collection list: each employee's monthly contribution
PhilHealth RF-1 Employer's remittance report: the premiums being paid
PhilHealth ER-2 Report of employee-members: new employees and changes

Employers file and pay on the agency's schedule. Check the current deadlines and channels with SSS and PhilHealth, since both have moved much of this online.

Generate them in OneHRIS

Open Reports, then Government.

  • SSS. Choose the period. The page generates the R-3 and the R-1A. The R-1A covers new hires and separated employees for the selected period, and the page shows how many updates it found.
  • PhilHealth. Choose the period. The page generates the RF-1 and ER-2, with each employee's salary base, employee share and employer share.

The reports are built from completed payroll for the month, so run and approve payroll first. See Run payroll.

Check before you file

  • The employer number and employee SSS and PhilHealth numbers are filled in. A blank number is the most common reason a submission is rejected.
  • Totals match the payroll register for the same month.
  • New hires and separations in the period are all listed.

Contribution amounts

The shares come from the contribution tables set in payroll. See Employer and employee shares of government contributions and Track government remittances.
